*Agreement of Policies

I understand and agree that: 

The information and all services provided such as, but not limited to, coaching, education, or consulting by the educator, Suzanne B. Yaeger, MA, LLC, and any information set forth on the educator’s website are to be used for educational purposes only and do not constitute professional medical or mental health advice or diagnosis, nor are they intended to. 

I am fully responsible for my (or my child's) well-being during educational services sessions and subsequently. 

Educational services are provided without expressed or implied warranty. 

I release the educator from any claim that may arise related to services. I have the legal ability to give my informed consent, and hereby give such consent to the educator to provide educational services to me (or my child). 

Cancelation Policy: If you need to cancel or reschedule, please text or email at least 24-hours in advance. Cancelations within 24 hours of appointment time will be charged at the full session fee. 

I am requesting educational services from Suzanne B. Yaeger, M.A., LLC. 

I, the client, understand and agree that by submitting the "Secure Payment and Policy Form," I have reviewed, understand, and agree to each of the statements and the terms and conditions stated above.
